Membership Program Guideline
- This membership plan is not insurance. It is only offered to individuals and families not currently insured by a dental plan (cannot be used with insurance). Plan is only valid at Aroma Smiles Dental. The benefits are not transferrable to another practice or dental specialist.
- It is the member’s responsibility to schedule and keep all appointments offered as part of the dental program.
- Membership is for 1 year beginning on the enrollment date. Dues are payable in full upon enrollment and are non-refundable.
- Cash, check or credit card payments are accepted.
- Payments for additional dental services are the member’s responsibility.
- Fees for dental services may change at any time without notice. Treatment plans are valid for 90 days after the date of the diagnosis.
- Please notify our office at least 48 hours in advance if you must change a scheduled appointment.
- No pre-authorization, no yearly maximums, no waiting period.
- Discounts cannot be applied to any other specials.
Aroma Smiles Dental Membership Plan Details
This is a dental membership plan and is not to be considered a dental insurance plan. The dental membership plan is for patients without dental insurance and cannot be used in conjunction with a dental insurance plan.
This plan may be used at Aroma Smiles Dental and applies only to dental procedures offered in this office.
There may be dental care needs beyond the scope of services provided at Aroma Smiles Dental. Patients with specialty dental care needs will be advised as soon as possible and referred to the appropriate dental specialists. Referred services ARE NOT COVERED by this membership plan. After specialty services are preformed, the patient may return to Aroma Smiles Dental and receive treatment covered under the Membership Plan.
Adults are $365 annually and children are $265 annually.
If periodontal disease is diagnosed, the member is financially responsible for the scaling and root planing (deep cleaning) at 15% discount. Member will receive 2 periodontal maintenance cleanings at no charge under the plan but will be financially responsible for any additional maintenance cleanings prescribed.
Membership dues are paid yearly and are due prior to services being rendered. Dues must be paid in full, and patient’s account must be current in order to receive benefits under the membership plan.
Notification of cancellation or renewal of the plan must be made 30 days prior to the renewal date. If the patient wants auto-renewal, patient must provide credit card information so that the system will automatically take the payment. There are no refunds for the subscription dues. Patients are not required to receive any recommended treatment that is diagnosed prior to using the benefits included with the membership.
This plan is non-transferable and can only be used for registered members of the plan.
Payments for services are due at the time services are rendered.

Adult Plan
Age 13 and older
$365 /Year
- Dental Cleanings 2X/Year
- Fluoride 2X/Year
- Routine Oral Exams 2X/Year
- All Necessary X-Rays included
- One Emergency Exam a year (if needed)
- 3D Scans included
- 15% of all additional services (restorative, surgery, cosmetic, etc. excluding Invisalign)
- $1,000 OFF of Invisalign
- $100 OFF whitening in-office
- Referral Services not included
